Terms & Conditions

1. Available for sale from October 29th, 2024, to December 31st, 2025.

2.This program is available exclusively for Thai nationals, expatriates with work permits in Thailand, and those with a Thai spouse.

3. Service Period and Access

3.1 For Basic Check-up group, the first service must be used by December 31st, 2025.

3.2 For Basic Check-up group, tests 2 to 21 can be performed an unlimited number of times, subject to the doctor’s discretion.

3.3 The test categories in Advance Check-up group can only be accessed after undergoing Basic Check-up.

3.4 The program can be used within 365 days or 1 year from the date of the first check-up.

3.5 For continuity in health monitoring, the next check-up must be with the same doctor who performed Basic Check-up.

3.6 If the service recipient is unable to receive the service by December 31st, 2025, they are still entitled to the first check-up by February 28th, 2026. However, the right to a follow-up check-up will expire on December 31st, 2026.

3.7 In case service recipients is unable to undergo the check-up within the specified time frame, the hospital reserves the right not to refund under any circumstances.

4. Program Check-up Service Fee

4.1 The mentioned price includes the doctor’s fee and service charges for the first and second check-ups only.

4.2 The mentioned price does not include the fees for consulting doctors from other specialized centers. In the event of abnormalities being detected and referral for further treatment is required, service recipients will need to pay additional doctor’s fees.

4.3 For the 3rd check-up and beyond, tests will be determined at the doctor’s discretion. Additional fees for the doctor’s consultation and other hospital services will apply as follows:

• • •  For follow-up reports related to specific lab tests, an additional fee of 300 THB will be charged for the doctor’s consultation.
    • For follow-up reports related to X-ray and CT Scan (1-2 tests), an additional fee of 500 THB will be charged for the doctor’s consultation.
    • For follow-up reports related to specific lab tests, X-ray, and CT Scan, an additional fee of 800 THB will be charged for the doctor’s consultation.
    • For follow-up reports related to Anti-aging category, an additional fee of 500 THB will be charged for the doctor’s consultation.

5. Advance Check-up Test Categories

5.1 For individuals who have undergone breast augmentation, if you choose to undergo a digital mammogram with breast ultrasound, kindly inform the staff before receiving the service. An additional fee of 1,000 THB will apply.

5.2 CT Scan examinations are subject to the doctor’s discretion.

5.3 If the doctor determines that a contrast agent injection is needed to detect abnormalities, the service recipient will be required to pay an additional fee for the contrast agent (the cost ranges from 8,000 to 15,000 THB, depending on the value of the contrast agent and medical supplies used during the procedure).

5.4 The Mental Health Program consultation lasts for 30 minutes with a psychotherapist. Kindly make an appointment at least 5-7 days in advance before receiving the service at Mental Health Center only or โทร.0-2617-2400 ext. 7448.

5.5 If the service recipient chooses to receive services under Advance Treatment category (Vitamin Drip), kindly make an appointment at least 1-2 days in advance at Premier Life Center, Phyathai 2 Hospital by calling Tel. 02-617-2444 ext. 3857 or 065-517-1889.

6. Changing the test categories in Advance Check-up group.

6.1 If a category has been selected for testing but not all tests within that category have been completed,service recipients can change the test category within 365 days or 1 year.

6.2 Once all categories have been completed, service recipients will not be able to change the test categories on the follow-up appointment date (the 2nd check-up).

7. The mentioned price includes a meal coupon and a report booklet for the first health check-up.

8. For your convenience, kindly make an appointment in advance by contacting Phyathai Call Center at 1772 (press 9 for ENG) (available 24 hours).

9. Once the program has been used, the rights cannot be transferred or the service recipient changed. The person receiving the check-up must remain the same throughout the duration of the program.

10. This program is for health check-ups and services at Outpatient Department (OPD) only. It cannot be used for check-ups or treatment at Inpatient Department (IPD).

11. This program cannot be exchanged for cash, other goods, or services, and no refunds can be issued under any circumstances.

12. For service recipients with a coupon for services, kindly present the coupon to the staff to claim your benefits before each check-up. If the service recipient fails to bring the coupon on the day of the service, the hospital reserves the right to charge the regular health check-up fee.

13. The program must be received at Phyathai 2 Hospital only. It is not possible to change the branch for service.

14. If the program is purchased online (via the website; phyathai.com, or Shopee), the service recipient will receive a receipt on the day of the health check-up. The receipt will be issued in the name of the service recipient, not the person who purchased the program.

15. The hospital reserves the right to make changes without prior notice.
